Oral and Generic Obesity Drugs: Can More People Stay on Paid Treatment at Lower Cost?

The two oral products launched in the United States in January and April 2026. Health Canada approved Apotex’s Sevmia on June 29, and the Medicare Bridge began July 1. Novo reports more than seven million Wegovy pill prescriptions in 2026 through its September presentation; Lilly reported $98 million of Foundayo revenue in Q2. These establish commercial activity and new access routes, not a count of persistent new patients.

Next review: October 29, 2026 · Terminal review: No fixed terminal date

The baseline records what was known at initiation. It does not assign a realized outcome or grade.

WHAT WE ARE TRACKING

Evidence to watch

Scope reviewed October 24, 2026 · Thesis evidence through 2026-10-22 · Edition 1

Keep product, country, currency, payer and cohort separate. Prescriptions are fills, not unique patients. No outcome grade is assigned; missing evidence is unknown, not a failed branch.

UNDER OBSERVATION

Foundayo product economics

Starting evidence
Q2 revenue was $98M; patient persistence, dose mix and unique new users remain undisclosed.
What would move the thesis
Review Q3 product revenue and any same-period dose/channel information. Company-wide volume or price cannot substitute for the pill’s figures; growth alone does not prove access expansion.
Next checkpoint
Confirmed Lilly Q3 results: October 29, 2026

UNDER OBSERVATION

Oral Wegovy access

Starting evidence
Novo reports high cash-led prescription volume; 9 mg and 25 mg combined is not the maintenance-dose share.
What would move the thesis
Review pill sales, script period, dose distribution and channel capture. Record naive-user claims with their lookback; do not infer incremental users from manufacturer shipments.
Next checkpoint
Novo nine-month report: November 4, 2026
Source record ↗

UNDER OBSERVATION

Maintenance-year affordability

Starting evidence
2026 Foundayo offers include expiry, caps and refill rules. Household all-in cost and payer net cost are not measured.
What would move the thesis
Record 2027 cash/card terms by dose, 45-day conditions, visit and membership fees. Keep the January Wegovy list-price change distinct from household and rebate-net costs.
Next checkpoint
2026 offers expire December 31; review replacement terms

UNDER OBSERVATION

Generic quality-released supply

Starting evidence
Sevmia is approved for weight management but its 1 mg pen cannot supply the complete labeled course alone. Reviewed recalls named other diabetes products.
What would move the thesis
Track product/DIN/lot-specific approvals, recalls, released doses, actual shipments, coverage and paid fills. Keep Dr. Reddy’s restart target separate from confirmed supply; retain Canada, India and U.S. liraglutide branches.
Next checkpoint
Regulatory and shipment events; no fixed release date

UNDER OBSERVATION

Pill persistence and substitution

Starting evidence
No twelve-month launch cohort has matured at the baseline. Cash and insurance records are not fully linked.
What would move the thesis
Measure maintenance reach and six-/twelve-month persistence using the stated 60-day gap, source coverage and prior-drug history. Identify switches from injectables or compounded products; label insured-only slices.
Next checkpoint
Earliest launch-cohort anniversaries January/April 2027, plus data lag

UNDER OBSERVATION

Medicare Bridge and other coverage

Starting evidence
CMS provides eligible users a $50 copay through December 2027; utilization is not published in the reviewed record.
What would move the thesis
Seek unique beneficiaries, prior-authorization requests and final decisions, prior use, persistence and spending if CMS publishes them. Track commercial and Medicaid changes separately; nonfill is not automatically denial.
Next checkpoint
Bridge cohort anniversary from July 2027; program end December 31, 2027

These are monitoring questions, not completed outcomes. Company targets retain that label; unscheduled disclosures have no invented deadline. Later observations must carry their own date, source and measurement scope.
